{"id":3035,"date":"2016-05-17T15:31:38","date_gmt":"2016-05-17T15:31:38","guid":{"rendered":"http:\/\/minutes.machine.market\/?p=3035"},"modified":"2016-05-17T15:31:38","modified_gmt":"2016-05-17T15:31:38","slug":"mesa-west-pty-ltd-fined-20000-over-the-death-of-allen-zuvela-at-fmgs-christmas-creek-mine-in-australia","status":"publish","type":"post","link":"https:\/\/machine.market\/blog\/mesa-west-pty-ltd-fined-20000-over-the-death-of-allen-zuvela-at-fmgs-christmas-creek-mine-in-australia\/","title":{"rendered":"Mesa West Pty Ltd fined over the death at FMG\u2019s Christmas Creek mine in Australia"},"content":{"rendered":"<p>A MAINTENANCE contractor has been fined $20,000 over the death of a worker at Fortescue Metals Group\u2019s Christmas Creek iron ore mine.<\/p>\n<p>Mesa West Pty Ltd was ordered to pay the penalty in Perth Magistrates Court on Tuesday.<\/p>\n<p>Allen Zuvela was killed and co-worker Ben Reid was seriously injured when they were crushed while working on a surface mining machine on December 29, 2013, because rigging between the cab and overhead crane failed due to overloading.<\/p>\n<p>WA Department of Mines and Petroleum Mines safety director Andrew Chaplyn said the department\u2019s investigation found a chain of events led to the incident that killed Mr Zuvela and injured Mr Reid.<\/p>\n<blockquote>\n<figure class=\"story-asset story-asset-secondary image-half\">\u201cThe pair were working underneath the cab of a Vermeer surface miner when the rigging\u00a0arrangement between the cab and overhead crane failed due to overloading,\u201d Mr Chaplyn said.<\/figure>\n<\/blockquote>\n<article class=\"story-content\">\n<blockquote><p>\u201cBecause of their position, neither Mr Zuvela nor Mr Reid could see the display for the overhead\u00a0crane load cell, which indicated the weight the crane was lifting.\u201d<\/p><\/blockquote>\n<p>Ms Chaplyn said despite the company\u2019s policy, training and instruction, which prohibited employees from working beneath suspended loads, Mr Zuvela and Mr Reid worked in the direct line of fire.<\/p>\n<p>He said there was also no specific procedure or Job Hazard Analysis (JHA) developed for the work Mr Reid and Mr Zuvela were doing.<\/p>\n<\/article>\n<div class=\"w_social-tools\"><\/div>\n<div class=\"w_outbrain-widget\"><\/div>\n","protected":false},"excerpt":{"rendered":"<p>A MAINTENANCE contractor has been fined $20,000 over the death of a worker at Fortescue Metals Group\u2019s Christmas Creek iron ore mine.
